Policies for Acupuncture with Insurance

If your policy covers our treatments, we will bill your insurance provider for our services. If our services are not covered under your plan, then we will inform you of alternate payment options for future treatments during your consultation, if not before..

Appointments with insurance not verified will be charged full price as an acupuncture treatment.

If you are approved, but must meet a deductible/have a copay, please note that the out of pocket cost for these appointments will be $110, until your deductible is met. Depending on your copay, you may decide not to go through insurance, and opt to pay $100 per appointment.

You will notice that the cost for Acupuncture with insurance will show as $110. This is the charge that will be incurred should you not make it in for your appointment without notifying NoteToSelf within our business hours 24 hours prior to the time of your appointment. Should you cancel your appointment within 24 hours, a $55 charge will apply. Your insurance deduction will be applied at time of checkout.

Your appointment must be confirmed with a credit or debit card. Please complete the Cancellation Policy and Traditional Chinese Medicine Consent Form when you confirm your appointment.

We accept cash, check, and most major credit cards. Cash patients can enjoy significant savings by purchasing a treatment package. Our packages include substantial discounts for pre-paying for your treatments. Regardless of your financial situation, we will tailor a treatment package if your insurance policy does not cover our services.
